To me, it means that you have only once chance to make a first impression and you should do your best to make sure that it's right the first time because that first impression sticks with you forever.
An example explaining the quote is when you meet someone important and your future depends on it, you want to make sure you are well-composed but if you introduce yourself poorly they will not think as well of you if you were professional.
